Laboratory Accreditation Policies

The laboratory accreditation policy modules are provided in both Word and PDF formats for your convenience.

The 2007 LQAP Policy Revision is now available. This revision will become effective April 1, 2007.  In addition, a cross-reference table detailing the changes can be found with the 2007 Module documents. 

Accreditation Application/Checklists/Guidelines/Forms

AIHA Site Assessor Checklist

In early 2005, AIHA updated the Site Assessor Checklist in order to adequately reflect the 2005 Policy Revision. AIHA’s updated Site Assessor Checklist now includes text taken directly from the ISO/IEC 17025:1999 standard in addition to AIHA requirements.  Due to the copyright restrictions, the Site Assessor Checklist is no longer publicly available through the AIHA website.

The LQAP is still committed to making the Site Assessor Checklist available to its accredited laboratories or other parties who already legally own a copy of the standard. To obtain a PDF version of the Site Assessor Checklist, you will need to send us proof of purchase of the ISO/IEC 17025 standard (either a receipt or a signed affidavit, which can be found in the on-line Accreditation Application). The revised 2007 site assessor checklist will be available by mid-February.  This checklist will incorporate the requirements of ISO/IEC 17025:2005 and the 2007 AIHA LQAP Policy revision.  A list of changes between ISO/IEC 17025:1999 and 2005 can be found here , and a list of changes in the 2007 Policy Revision is available here. Please continue to check this page for a notification of when the new checklist is available.  An electronic copy of the revised checklist will be automatically sent (once completed) to all labs that have returned the ISO/IEC 17025:2005 Affidavit document that was e-mailed to all accredited labs in October 2006.  If your lab has not received a copy of this affidavit, it is available online (please write an updated e-mail address on the affidavit).
