Below are commonly requested forms for DL students. The list excludes TeleWebs as transfers and extensions as they do not apply to TeleWebs. If you can not access the URL to submit a TeleWeb evaluation or if you have questions, please contact the distance learning assistant at dlassistant@aiha.org or call (703) 849-8888.
- Transfer Request Form
: Students who enroll in a course may transfer the rights of that course over to someone else.
- Course Extension Form: Students may extend their deadlines up to one year after the original expiration date.
- Course Evaluation Form : Evaluations must be submitted along with course requirements to earn credit for distance learning courses.
